Registration and Forms
A completed Registration Form (including credit card information) along with the signed Placement Agreement Form are required to be placed on active file. Please return the two documents to the address or fax number listed on the bottom of the form.

Both the registration and placement agreement forms are required in order to begin a search for your child care provider.

The Client's Emergency Information Form is kept on file should the provider have an emergency situation and be unable to access the information from your home.

4. Depending on your browser, the file may open automatically in Adobe Acrobat. If not, save the file to your hard drive and open it in Adobe Acrobat.
5. Print the form, fill-out completely (be sure to sign and date the form) and mail to Child Care Professionals, Inc. (address is provided on form).
